so with incapsula the HideProxy is totally messing up, it randomly displays IPs, like this:

So as you can see the displayed IP is not the real one, the real one is the one sent in x-forwarded-for and incap-client-ip.

Is 91.45.250.249 the IP address of the reverse proxy?

It's neither, it's some ip address of another user browsing the site.

I tested the reverse proxy again. It works fine for me. I have no idea what goes wrong in your setup.
