php script fails intermittently

Hiawatha version: 7.2.3
Operating System: OS X 10.6

I have a simple captcha script in php embedded in an html page that displays a four-letter code. When I start hiawatha the page loads fine and I presume the captcha script runs as well since the captcha is displayed on the page. Page reloads also change the displayed captcha. However after some time, the captcha is not displayed, even on page refresh, or clearing browser cache. The only way to bring it back is to restart the hiawatha server process. There are no entries in the log files. Any ideas?
